On day 13, I have a fruit 27 inches in circumference. A leaf and secondary vine are attached to the stem. Do I cut the leaf and secondary above the stem?

The secondary should go before it interferes with the fruits growth. Likewise the leaf so it doesn't scratch the skin. But take your time. If the fruit aborts, you'll be glad you still have the secondary. And for now, the leaf might be offering shade. Just make sure if a gust of wind pinwheels the leaf, it can't reach the fruit. If it can, chop it off clean & close.
