P is really high. K is roughly right (slightly high). You need 3000-4000 Ca. The base saturations are great for Na, could go up a percent or two for K, Mg is good, and Ca could go up to 80.

Maybe add a little lime to bring the pH up. Actually, go with some calcitic lime. Would take care of pH and Ca at the same time. I would let someone else chime in for application rates though.

I would add a bunch of leaves (maple is preferable) and grass clippings for OM, but as far as manure goes, adding some will make your P and K even higher. Phosphorous should really be under 100, but definitely not more than 200 I would say.

Phos is on the high side, but there is very little you can do about that. I would add a bunch of leaf or aged mushroom compost if you can get it. I wouldn't worry a whole lot about the PH at this time, although as you add OM it will likely drop some more. The K level can come up as well. I would add your compost, then retest. If your K level is still near this level add Potasium sulfate in the spring at 8-10 lbs /1000ft2. As your k comes up watch your k:Mg ratio, keep the Mg above the K, but no more than double. You may wish to add Calcium, by way of lime, calsitic or dolomitic could be used or some gypsum as the PH is really not a problem. If you add mushroom compost, it is usually PH adjusted with lime.
