after talking on the phone with the lab, 3/4 lb. of nitrogen is what he would recommend. so with that i figured-
5.7lbs of 13-0-0 blood meal
8.7lbs of 0-40-0 triple phosphate
2lbs of 0-0-50 potassium sulphate
20 lbs dolomite lime
.5 lb of sulfur
does this look right or should i do something different ?

might want to change the tripple phospate in to a more organic approach like bonemeal, or rock phospate.

What did you add to get the organic matter up so high? That will affect the N amount I'd recommend, unless you're "organic" urea is cheaper.

It's a little late to lime (fall is better) and your pH isn't terrible, but if you want to lime, just for a Ca and Mg boost, I'd do that right away, and wait at least 2-3 weeks before adding everything else. Do a quick light till during that interim time if the soil dries out enough. Definately don't add lime and P together...and you need to add P worse than you need to lime.

I'd look at DAP Diammonium phosphate (18-46-0) as a P (and N) source. It tends toward higher pH.

You don't need any elemental S. The potassium sulphate
will cover your S.

Also a Tablespoon of Borax for the low B wouldn't hurt.
