Hi all, I think I could have a good 2-4 pound tomato, but it looks like there is not a physical form. I remember last year I had to have the scale operator sign a form. Any help would be greatly appreciated. Thanks, Leif

It asks for tomato info, witness info and the weighing info. This is the way to enter now, correct?

Their is no paper form anymore just this on line form.
Be prepared when filling out the form it must be completely filled out or it wont let you submit it.
I only did a test run with my 6.23 DMG Domingo last year but you need pictures of the tomato on a certified scale at a store a picture with a tape measure around it and a few of top, bottom, of tomato and a picture of when the scale was last updated. Also need 2 witnesses with their email and contact number and your email and contact number.
Last year my 6.23 was dammaged so i weighed it on my postal scale at home then sent it in and then they removed it from the gpc list 2 days later ? My guess it was because i did not weight it on a store certified scale ? But because it was leaking i was not going to take it to a store and have it leak on their scales. Just a warning you to fill out the form correctly.
