This came back from A & L Labs in Ontario
Can anyone Give me any recomendations?

They told me 5 lbs of Nitrogen per 1000 square feet
2 lbs of phophorous
2 lbs of potasium
1 lb of Mg

Is the K really 1409 or is that a typo and maybe 409?

No. The potassium is 1409 ppm

I wouldn't add P or K. The only things I'd consider adding would be gypsum and some nitrogen. What are the base saturation %'s?

I think you'll be fine but you could add some gypsum to bring the Ca up a little.
