Organic Matter 2.7 Phosphorus p1 81 ppm p2 135 ppm Potassium 172 ppm Magnesium 378 ppm Calcium 2091 ppm Sodium 26 ppm Ph 7.5 Cec 14.2 Sulfur 57 ppm Zinc 8.5 ppm Manganese 7 ppm Iron 42 ppm Copper 1.5 ppm Boron 1.2 ppm
I already added 10 inches of aged manure 6 inches of maple leaves and sulfer to lower ph.

If the test was taken before the manure, work it all in and wait until spring, then retest. Was the manure well composted? That sounds like a lot of material.

Besides the sulfur your gonna have to add extra pottasium.

You should of just tested the manure... That is what you'll be growing in. I would have a plan in place for root disease. Your numbers should however easily triple.

The manure was high quality or at least it looked that way and it was well composted. How do you add potassium?

You wont need any potassium... Hold what you got.
