Start off with lots of compost and manure. Lime to raise the PH.

I'll give my 2 cents. This was my first year so I'm still learning.
Everything is extremly low. add 6 to 8 inches of composted manure this fall to your soil. PH and Ca will still be low after this. Have another soil test done in the early spring this will tell you what to use to raise your Ca & PH. Read everything you can get your hands on about "soil web" there's too much to learn this should get you started.
