It would appear you have milldew and SVB. The mildew can be treated with a fungicide. I would prefer one containing copper. Move quickly on this and apply late evening or cool of the early AM. If in the high heat of the day you can burn the leaves.

Digging out the bore of SVB is OK. I am leaving this part of your answer up to others. Without help I would immediatedly apply foliar application of Merit per label instructions for leaf feeding. It will travel one foot per hour and could do in remaining or new bores for you.

In either event time is important. You are already behind the eight ball. I can't tell you where to get Merit quicly.

Grande, welcome to the club. As Doc suggested, Daconil or
a copper based fungicide should be administered quickly to arrest the powdery mildew. Neptunes Harvest fish and seaweed emusions work well to prevent mildew I have found.
We sweated through two weeks of high humidity here in NE and mildew is everywhere. I've used an eyedropper type syringe in the past to inject Sevin into suspect borer holes with pretty good success. If you have any other ???
feel free to email me as I'm not too far away...
